README: update GAP entries in table

As well as adding a few entries that were not present before, I've removed the directory-based cd names for GAP. Markus and I talked about how GAP organises functions, and it probably doesn't make sense to do it by folder so much as by package. Hence all functions in the GAP library just use "lib" here.
